A Girl Shortage Form should be submitted if a Girl Scout and her family does not pay for product they have received. Girl Scouts of Greater Iowa will then contact the family to make payment arrangements. Troops should make at least two collection attempts prior to submitting a Girl Shortage Form.
Before filling out this form, please be sure you have scanned copies of the following documentation to attach after completion:
- Signed Product Program Permission Form for Girl Scout about whom you are filing the shortage. The Girl Shortage Form will NOT be processed without the signed Product Program Permission Form.
- Initials proving acceptance of product/copy and receipt.
**Troops will be credited for the amount of the shortage only if all requested materials are provided by the deadline. If the deadline is not met and/or no signed Product Program Permission Form is provided, the troop will be credited only after Girl Scouts of Greater Iowa is successful in collecting shortage amount. **
The Fall Product Program deadline to submit the Girl Shortage Form is November 1, 2022.
The 2023 Cookie Program deadline to submit the Girl Shortage Form is April 7, 2023.
